Welcome to Modiv Health PLLC (“Modiv Health”). By using our website and services, you agree to the following terms:

1. Telehealth Services

• Modiv Health provides virtual medical weight loss services including consultations, GLP-1 prescriptions (when appropriate), and lifestyle coaching.

• Services are available only to patients physically located in Utah at the time of consultation (unless future licensing allows expansion).

2. Eligibility

• You must be at least 18 years old to receive services.

• You must provide accurate and complete health information.

• Final eligibility for medications (including GLP-1s) is determined by a licensed provider.

3. Not Emergency Care

• Modiv Health is not for urgent or emergency medical needs. If you experience a medical emergency, call 911 or go to the nearest ER.

4. Payments & Refunds

• Payment is due at the time of booking or as outlined in your package agreement.

• Refunds are not guaranteed once a consultation has occurred, but unused services in prepaid packages may be refunded at our discretion.

5. Patient Responsibilities

• You agree to follow treatment plans as prescribed.

• You agree to use prescribed medications as directed and not share them with others.

• You agree to keep your login and health account information secure.

6. Technology Requirements

• You are responsible for having a stable internet connection and compatible device for telehealth services.

7. Privacy & HIPAA

• Your information is protected under HIPAA. See our Privacy Policy for full details.

8. Limitation of Liability

• Modiv Health and its providers are not liable for indirect or incidental damages.

• Care is provided to the best of our ability, but no results (e.g., specific weight loss outcomes) are guaranteed.

9. Changes to Terms

We may update these Terms. Continued use of services means you accept any updates.
